How it runs

Four stages · One file

Every investigation runs the same four stages.

Terms first, then evidence, then findings, then a report written to be read by people who were not in the room. Nothing is decided before the last stage, and each stage carries a standard we do not move on.

Turnaround

Terms agreed inside five working days, before anyone is interviewed.

Evidence

Documents first, then people. Everyone who is named is told what is said about them, and gets the chance to answer it on the record.

Interviews

Recorded and transcribed. Every account goes back to the witness for correction before it is relied on.

Fact by fact, on the balance of probabilities, with the reasoning set out — so a reader can see how each conclusion was reached.

Standard of proof

Balance of probabilities, stated finding by finding, with the evidence each one rests on named.

Written for the people who will test it: a tribunal, a regulator, a board. Not for whoever commissioned it.

Disclosure

Written on the assumption that it will be disclosed in full, to everyone in it. Six to ten weeks from terms to delivery.
